What is a personalized form?
Personalized forms are completed by entering the required information
on an entry screen. After clicking the "Create
Form" button, this information is transferred to a PDF
format along with a scan line, which is located in the bottom
left hand corner. This scan line allows the Department to efficiently
process your return.
Do not print the entry screen. Click the "Next"
button and wait until the PDF form is generated before printing.
What are the software requirements for using
personalized forms?
- Adobe Acrobat Reader version 5.0 or greater
- Browser Requirements:
Netscape (versions 4.7 or greater) or Internet Explorer (version
6.0 or greater)
How do I find out which version
of Adobe Acrobat Reader that I use?
Locate the Acrobat Reader program on your machine and open the program
(you probably have an icon on your desktop for Acrobat Reader that
you can double-click). Click on the "Help" menu item at
the top of Acrobat Reader window. Select "About Acrobat Reader"
from the menu options. A window will appear that displays the version
(ex: Acrobat 5.1.0).
How do I find out which version
of Internet Explorer or Netscape that I use?
Click the "Help" menu item at the top of the browser window.
If using Internet Explorer, select "About Internet Explorer"
from the menu options. If using Netscape, select either "About
Netscape" or "About Communicator" (depending on the
version that you have). A window will appear that displays the version.
I clicked on
a "personalized form" link, but I get a page that
says
"The Page Cannot Be Found" or "Not Found". Why
did this happen?
The problem is most likely related to your browser not being 128
bit encryption. When you click the "personalized form"
you are moving to a secure server. To see the web pages on the secure
server you need to follow the browser
requirements.

the latest version of the free Adobe
Acrobat Reader.

The personalized forms are not accessible during our maintenance period each Sunday from 4:00 a.m. to 12:00 p.m.
